Physician Information
To obtain a license as a homeopathic and integrated medicine physician the State of Arizona requires the completion of an application and the required examinations.
Examination Dates (Please Note: Examination and Meeting Dates may change. For instance, if an examination falls on a State holiday the examination will be held on the Friday before the holiday). Contact the office for more information. 602-542-8154.

Continuing Education (Each physician shall indicate on the annual renewal form in a statement that 20 hours of board approved continuing education was completed in the previous year).
